My childhood dentist never used Novocaine except when pulling a tooth. I thought this was usual at the time. Seems it is not.
I had a baby tooth pulled at the dentist that didn't have an adult tooth to replace it. Had two more perfectly good teeth pulled by the oral surgeon before I had braces. Then had my upper wisdom teeth pulled when one broke like your tooth did.

My advice on looking for a dentist:
1) you want one who STANDS next to the chair
(Better reach, less stretch, better view for the dentist - more efficient treatment)
2) Choose one who offers nitrous oxide - if you're spazzy at all at the idea of dental work, it's a MUST. If you hate pain, it's a MUST (it has some painkilling properties, but more important is the relaxing while the noise is going on)
3) Plenty. Of. Novacaine. The dentist must believe in painkilling first, rather than "suck it up and deal, princess". You get any dentist with the latter attude? Run, don't walk, the hell out of there.
Anyone who's advertising a LOT? Probably isn't one you want to go to. A good dentist, honestly, won't NEED to advertise.
