does that mean we see her sporadically in flashback, or he sees her frequently as part of his traumatised delusion?

I'm hoping the latter as well. Since her character hasn't been described explicitly as a delusion, I think what they're going to do is introduce her as his wife, within the context of his home, and later reveal that she's yet another of his multiple personalities.
