Does anyone have any experience in appearance contracts?

I have what I have to offer and what I ask for in an easy to read document, I just am having trouble converting it to something that is legal and standardized.

I could take a look. I'm not terribly familiar with appearance contracts specifically, but I draft all our contracts before sending them to our lawyer for review, it saves a great deal of money and so far has needed minimal changes. It's really just being logical and asking, "How many ways can someone else fuck this up?"
