There are dozens and dozens of Omegas and Betas in population but only a handful of Alphas, who are considered royalty/have elevated positions in the community -- they govern it. When Omegas go into heat (something that happens once a year?), unmated, virgin Omegas are presented to the Alphas to pick and choose from as a from of "mandated peace offering ceremony", for the community to exemplify their "thanks". Politics, etc.
Alpha!Derek chooses virgin!Omega!Stiles, to Stiles' absolute horror.
