Wow this was haunting the confrontation between Sam and the demon inside the girl was amazing!
I adored this line:
Sam grinned, focusing hard on the demon, dark satisfaction curling deep in his belly like warm drowsy wine.

Quick comment before I go to the next part. LOVE!!! that the girl realizes that Sam saved her and tries to protect him before she understands that Dean isn't a threat to him. And this bit:
looking into those eyes she'd never seen anything more beautiful. Or anything more tragic. Oh, Dean! Now going to make sure that Sam's okay!
