I seem to remember that I needed to bring a letter of acceptance from my school, and a passport, and then fill out a form at the Japanese consulate in Seattle.
Then, they put some stickers in your passport. It is a stamp good for XX years, that allows you to get a visa upon entry to the country.
The visa itself (in my case) is just a tiny sticker that the guy at Japanese customs put in my passport.
