Decimated means "10% destroyed", not "obliterated". It's right there in the first two syllables, people, figure it the $%^# out already.¹
"Phase" is what Geordi does to the injector coils when they don't work right. "Fazed" is what J.Shep is when a space princess wants to get it on. Yes, it really is spelled that way.
Yeah, I was totally delighted when I saw/heard that in the ep, and I'm pretty sure I made a point of noting it in a subsequent LJ post, too. :D
Phase/faze is one that's driven me crazy for years -- it's one thing (still irritating) to see it in fic, but I've also seen it repeatedly in (non-tabloid) newspapers and other sources which should really know better.
Oh, that bugs me too! So many people seem to use one or the other exclusively, when they're both right in different contexts.
It's funny: I can deal with lolspeak, which totally butchers the language, but these sorts of (relatively minor) errors in usage annoy me so much more, probably because I almost always see them in contexts where I expect a high level of discourse.

Me, I blame the tabloids.
And yes, nothing more annoying than writers who can't tell the difference between "phase" and "faze" - alas, some of those are pros.
