Hilary becomes an eye surgeon, taking part in many lovingly detailed operations. Then she quits medicine in order to stay home and give Julian the big family he's always wanted. Perhaps this was a reaction to Lisa's tragic yet noble death in childbirth. It's sort of healing procreation. Also there is quite a bit of lactation kink.

She leaned forward, ready to make the first incision. "Besides," she said, "if this goes wrong - we have so many more."
John is molesting Sam, and Dean finds out after seeing how Sam is so emotionally devastated. Dean must deal with John and find a way to help Sam deal with the abuse and heal. Sam/John and eventual Sam/Dean.
