I. STATS
NAME: Please list full name and/or any aliases they may have/have had at one point.
AGE: Exact, or an estimate, if possible. If there is a discrepancy between actual age and bodily age, you need tell us both. The physical age will go into the dossier.
HEIGHT: Make a good guess if you don't have canon material to work with. This is for the patient files and dossiers, so in-game only.
WEIGHT: Same as above, make a guess if you have to. This may be helpful. Remember the game takes place in Switzerland, 1892 - the official IC system will be metric, which was adopted in Switzerland in 1835. Don't worry, though - you only have to give us either metric OR Imperial, we can convert it ourselves. Furthermore, for the convenience of our non-metric players, both metric and Imperial will be mentioned in the dossier. Your characters, however, will only see metric.
SEX: Yes, Hermaphrodite counts.
NATIONALITY: Pick one. Your nationality is your citizenship. Doesn't matter what ethnic mix your character is, this is what you would call his home country. And yes, really, only one counts. For all those who play characters from worlds different from our real world, and all those whose characters don't have canon nationalities, etc.: Don't worry. You're not supposed to give a canon-representation here, unless there is one available (if so, it'd be cool if you went with it, but not absolutely required), but to just name what the NPC staff would assume where your character is from. You can choose relatively freely. Make sure it somehow fits with the character's name (family name is obviously more important), spoken languages, family relationships (try to be the same nationality as your brother/father/wtf-ever, for instance, if they are already in the game). Otherwise? Feel free to use this to mess with the character, choose something that'd upset them, or whatever you like.
II. CANON
FANDOM: If your fandom has different versions (novels, manga, anime, series, movie, etc.), please clarify which one you are taking them from. We cannot allow any mixing of the different canons, unless they don't have discrepancies.
TIMELINE PERIOD: Please clarify which point in the course of the story you are taking them from. Canon-updates are possible, after discussion with the mods, but should be kept at a minimum.
ABILITIES: For information on the restrictions placed on superhuman/magical abilities in St. Edelweiss, please see here.
If your character has superhuman/magical abilities in their canon, please list (a) what these are and (b) how they will be limited in Edelweiss. The exact nature of the limitations placed on powers is largely up to you, within the guidelines listed in the FAQ.
If your characters has no superhuman abilities in canon, list (a) whether they will be gaining an ability when they enter Edelweiss and (b) if so, what that power will be. Ideally, such powers should be somehow related to the character's canonical skills/personality.
For all characters, please list any noteworthy, non-magical skills they may possess, e.g. martial arts training, lockpicking, and so on.

III. CHARACTER HISTORY
You may either write something up or link something either written by you or by a third party. However, you should be aware that we aren't asking for a canon summary here, but for your character's personal history. Mention everything that is important. You don't have to ramble about tiny details, but please give us a solid background and description of the character's development. Two solid paragraphs are the absolute minimum.
IV. CHARACTER PERSONALITY
No linking here, please write something up - exception is, of course, if you link to something that you wrote yourself. Be as detailed as you wish, but please try not to fill this with too much headcanon, rambling, and unimportant things. We're looking for a description of their personality, of their motives and what makes them tick. Three solid paragraphs are the minimum.
V. IN-GAME
DANGER LEVEL: 1 or 2. As a guideline, 2 is only for violent, overly aggressive, or suicidal patients. While clearly giving your character more street cred, the cells are tiny and suck. The food sucks, too. :) And naturally, the freedom is a bit more restricted.
WHY DO YOU WANT TO PLAY THE CHARACTER IN THIS PARTICULAR GAME AND SETTING?
Here, you can tell us anything. Why do you think the character is a good choice here/will work well in the setting? How are they appropriate for it? (This is particularly important and required to be explained if you apply for a character from a kids' show, for instance, or any other character that might be a bit of a weird choice at first glance, for a setting so dark.) What chances do you see for yourself here, what do you want to play out, how can you see them reacting and developing? What interests you in the setting itself? Note that these are, unless otherwise mentioned, all just guidelines. Here, we want to know what your motivation is, and what you are looking forward to and seeing in the future, playing them here.
WHAT WILL THEIR DIAGNOSIS BE?
Four diagnoses maximum. No, your character does not suddenly start to suffer from them when they arrive here - it's just the staff's way of re-interpreting your character's dominant personality traits and quirks as pathological. Make it fit in with their personality and background, but don't play the disorders out, unless your character canonically suffers from them. If your character suffers from another condition (illnesses, suicidal tendencies, a drinking or drug problem, gambling, etc.), please mention that here additionally to your diagnoses. These things will receive a special note in the dossier.

Note that you do have to write enough for us to tell that the character's voice is fitting. Merely silence because they are secretive does not work. Make up an imaginary situation if you have to, but show us that you can voicepost and have interaction ICly with an appropriate character voice. If you have a creative idea for another kind of staff position that we haven't listed yet, there is a chance that we can work something out with you. In that case, though, please contact the mods before sending in your application.
TREATMENT METHODS: What is your character's general idea of how to treat their patients? What fields of research are they interested in, and how will they usually perform their duties? We're looking for a general explanation here, and you are encouraged to use your imagination and ramble a little. Are they more likely to try to gain the patients' trust and learn things about them to control them? Are they more interested in confronting the patients with their deepest fears to heal them? Do they like the assistance of drugs, other medical procedures, or do they work mainly through the patient's mind?
SPECIALIZATION: Just one or two words (okay, or three - you get it). Something that summarizes what could be called your character's special field, in contrast to the other doctors.
