Different Roads. The first post it the master post. I haven't read the entire series. Dean raises Sam away from John starting Sam's 6th grade, I think. No Adam. Also Dean/OFC and Sam/OFC
Brother Mine. Dean gets a call that Kate Milligan is dead and John needs to claim Adam. Sam and Dean go out and meet him. Dean raises him and Sam.
For 4)
My Brother. Dean is injured on his way to visit Sam at Stanford. He stays with Sam while he recovers. (This story has the most accurate portrayal of retrograde amnesia I have ever read.)
Unexpected Consequences Dean is sick and stays with Sam and Jess. Short but sweet.
For 5)
I don't really know any with telepathy and most of the ones I like are crossovers. So here are a few that I hope you like:
Wolverines, Windigos, and Winchesters. (Empath!Dean, and het) Dean meets Wolverine on a hunt that turns out to be a Wendigo and later Professor X offers him a job teaching at the Institute.
