sounds like Open, Shut by Mirrordance. Even if not, it's a terrific story, and can't recommend everything written by the same author highly enough.

Oddly enough, I just read this fic yesterday. I guess it fits your #4 in that Dean is trapped and hurt, and Sam is working to save him.
