Can't recall if it is post purgatory or not, BUT

- I recall somehow, Sam was actually tortured for a couple of days or so by Dean on the rack. There might have been fucking involved.

- Sign is that there is a mark behind the ear. Dean checked when he got back for the mark, and he was relieved it wasn't there. BUT, Sam actually just hid it

I know it's not the one you're looking for, but if you wanted something similar, Three Days on the Rack has many of the other story's elements (Dean torturing Sam, non-con).
