Another option: Where Do We Begin (The Rubble or Our Sins?) by dimeliora I love this story! A great read even if this isn't the one you're looking for.

Summary: AU post season one. He wakes in a hospital, missing a limb and all of his memories. They tell him he is Dean Hunt. That his sister died in the accident that claimed his leg. From there he rebuilds with the help of his quirky lesbian neighbors and a homeless drifter named Sam.

Memories of Me by Gillian fits this description - it was first posted in 2006, I think and it’s amazing and unique in that they don’t end up “together”, which is unusual in these types of fics, I think.
