Not sure about your #2 - the committing suicide part to get some rest sounds like Here We Still Remain, but it isn't gen, and although Sam is exhausted, the actual injuries don't carry over.

I don't think this is what your talking about but for #2 theres this one: Everyone dies given time.
Basically Sam dies on a hunt and gets bought back by lucifer, he doesn't really care anymore so he just keeps putting himself in danger eventually Dean catches up with him.
