It's the RNG screwing you over. Keep trying, and if you get really desperate, go to the mall (if not Ronin or HC).

Is it only the mosquito that can get the lead necklace? I haven't had this problem with any of my other familiars.

most certainly not ;)

The lead necklace is available if you go to the cake-shaped arena for ANY familiar...it's a matter of rotten luck to get it though.

having one or two isn't bad :-) but yeah - if you use the arena a lot, you're going to get a lot more lead necklaces than you can use.

Whenever I equip my lead necklace before going into the arena, it gives me the good equipment, maybe I'm just lucky.

Jick's said that it's a 75% chance of getting familiar-specific equipment, and a 25% chance of getting a lead necklace. It's just luck.
