It is a shrine festival, and it does exist.

I don't know about the end-of-year race, but I did attend one in Fukuoka, western Japan. I had to wake up at 4 am to arrive at the shrine on foot by 5 am.

Only men, of any age - from little boys to old great-grandpas, took part. They did indeed wear that Fundonshi traditional underwear. And folks lining the streets doused the running men with water.

Groups took turns to carry the gigantic shrine as the whole procession made its way across town. Roads were of course closed to traffic for that event.

It's actually a fanart of Kazuya Minekura's manga series Saiyuki, which is a more modernized, pretty-boy take on the Chinese legend of the Journey to the West and the Monkey King. The blond is the Buddhist priest Sanzo (Xuanzang), while the brunet under him is the Monkey King Son Goku. (The, uh, guy under the bed is another story--he was a sort of one-off plot device character created for the anime, and who wanted Goku for himself.)
