She knew he wouldn't really hurt her? Sorry, but if you 'let fly' with a belt, as the description in the fic says, and it hits bare flesh with a 'sharp crack', then that is going to hurt. More than enough to bring tears to the victim's eyes - and even more so if it's without warning.

I know that pain as part of sex is a legitimate kink, so I'm certainly not objecting to that per se. I think I, personally, would have appreciated a BDSM warning so I would have avoided reading this - I was enjoying the smut right up until the violence, which in any context is a huge turn-off for me.
