Hmmm...that is an interesting question. If Cas raised Sam up bodily then where did Lucifer go? I suppose this will be a "never really know" type of thing, unless they address it next season. My idea is that Lucifer wore Sam down there, but his body got destroyed, we've seen them reform rather dilapidated corpses on the show before, so Cas just pulled the bits together again and thus Sam and Lucifer were maybe down there but no longer joined together, it was a spiritual realm that didn't rely on bodies. So, Lucifer, Sam, Michael, and Adam were all separate beings, with Sam and Adam probably being pushed around by the big guys while they duke it out eternally. I mean, Lucifer needed a vessel on earth, he didn't need one in hell. So, that's what I think happened, Cas did the easy thing first, remade Sam's body, but it took Death to go to hell and get his soul and remove it from the clutches of Lucifer, Micheal, and hell, a much tricker task. Just my two cents. :)

But wouldn't that be a bit crowded in the cage, to have Michael, Lucifer, Adam and Sam. Unless Adam wasn't in there. We weren't given a hint to his whereabouts. Do you think he was in the cage with Sam or was Sam suffering Michael and Lucifer's torture alone?

Depends on if you think that it's a physical place or a spiritual one, and I prescribe to the latter idea, thus space has nothing to do with it, it could be the size of a pin's head and they could fit a million souls/beings/spirits there. Thus, Adam would still be there, because didn't Death ask Dean which brother he wanted pulled out of the pit and that he could only do one or the other, or am I mistaken in that?

I've always believed that once they were back in the cage, both Michael and Lucifer abandoned their vessels. They only needed them to fight on earth. Lucifer was in the cage in his Archangel form for just about forever so I don't see why he would want to stay in Sam's body once he was caged again. I also think that the cage was meant to hold archangels not human bodies or souls, so Cas was able to pull Sam's body out and Death was able to get Sam's soul. It's like mice and rats can get into a prison cell because they are so small, but the people in the cell can't get out.

Hm. That would make sense. But was Adam in there too. Cause there was no indication that he was or he was suffering what Sam was suffering. Unless they aren't telling us now and we'll find out next season?

I have no reason to think Adam isn't still in the cage. Dean asked Death to get Adam out and Death said no. Cas didn't care to get Adam out, Dean chose Sam over Adam. Poor Adam is collateral damage in the Apocalypse. He is either suffering what Sam suffered or Michael is honorable enough to protect Adam as much as he can, whereas Lucifer had no reason to protect Sam once he pulled him back into the cage. If Adam is still in the cage, that's just more survivor's guilt for Sam.

Hm. I wonder if the cage was built to hold any Archangel or just those who fall. I only bring this up because if the cage was specially made for Lucifer then wouldn't Michael be able to at least set a human soul free, or even yet, bust himself out? I really don't see Michael protecting Adam at all, he didn't seem to care for Adam when he was possessing him.
