I think it is a waste when you're talking to enemies who might be acting like your friends.

But if your enemies are acting like your friends, then your friends might be acting like your enemies' friends, so anyone acting like your enemy might be your friend. So it's never a bad idea to be polite.

Uh, I recommend seeing a doctor, then. Megumi-san is good, or maybe the hospital if she can't, I actually don't have much experience with this stuff.
