This prompt got a nice mini fill, but I think it has the potential for an interesting story too.

The case is closed, the murderess is about to be carted off, the good guys won... and suddenly the girl flips out and lashes out and almost kills John.

Sherlock goes spare and demonstrates that the diagnosis of sociopath is right by having to be physically restrained by a shitload of policemen from more or less vivisecting the murderess where she stands.

For some reason, Lestrade and co. are a tad horrified at seeing this level of murderous rage (esp. against a girl) and having Mycroft appearing from thin air to collect his brother all bland and nonchalant doesn't really appease their worries.

Namely: just how bare is the thread holding Sherlock to sanity and what role does John play in that? Also, why is his brother so blasé about it and how much power does he wield?

Some time after John, Sherlock, and Jim all escape from the situation at the end of Episode 3, Moriarty is desperate for Sherlock's attention again. He sets up an elaborate Alternate Reality Game (link 1, link2) with clues to his whereabouts/crimes in URLs/websites/RL/file names/mysterious YouTube videos, etc. etc. It's a multimedia event, but with crime.

Any pairing, but I'd really love to see Mycroft and Lestrade involved as well.

Set when Sherlock and Lestrade start working together. I'd like to see a story that explores their first meeting, Lestrade discovering Sherlock's drug addiction and possibly helping him get off it (I've always thought that nicotine patch scene could hint at some 'pact' they made years ago, but maybe that's just me) and working together.

I'd like it if it's slash, but that's really all up to you, writer. :)

Sherlock develops a medical condition which in general can be controlled but has the potential to be life threatening (like asthma or severe allergy or something). And of course you know Sherlock would never look after himself properly or let it slow him down on a case (eat? Why should I eat? Who cares what my blood sugar is. I can't be expected to remember my inhaler when I'm chasing criminals. etc.)

Bonus points if he has a severe reaction when John is unavailable and someone unexpected knows exactly what to do (Lestrade, Anderson, random stranger who turns out to be a stalker Mycroft put on his brother to keep him safe...Moriarty jumping out from behind a bush screaming 'You can't die before I kill you!')

Slash would also be excellent. Lot's of h/c and maybe Sherlock discovering that even if he doesn't look after himself for himself, maybe he should for others who care about him. So...is the prompt vague yet specific enough? Really, I'll go for anything with Sherlock being sick/hurt and others angsting over him/comforting him.
