The reason he was so terrified at Kitty's flat was because he knew he had to get hints of his brother's personality out so it would seem like he was acting. He knew if he didn't his husband would die and if Sherlock caught on he would as well.
One day John and Rich meet in a casual setting and Rich is instantly scared stiff because he thinks his job isn't actually done yet (because Jim never actually said it was) or that John will actually murder him right in public.
