Sherlock survives the fall but, recovering in hospital, it's obvious that he's severely impaired as a result. John, posing as his partner so that he can claim to have at least some rights over Sherlock's care - a right that's significantly beefed up by Mycroft - vows to take care of him.

John has to face the fact that Sherlock isn't as brilliant as he was anymore.
