Because I've seen this in just about every fandom I've been in, I just had to do this. Both John's parents and Sherlock's Mummy have been hassling them about marriage and grandchildren, which neither are particularly concerned with at present. While grousing together about nosy mothers, John admits he'd like a child or two of his own someday, and Sherlock notes that it would only be right for him to sire an heir to carry on his work--John takes that as agreement. Unbeknownst to him, the conversation sparks Sherlock's imagination and he makes a proposal. In (whatever) years' time, if neither Sherlock nor John are in a relationship, they should get married and/or have a child together to appease their mothers and fulfill the biological imperative to reproduce. John doesn't think it'll ever happen, so he agrees.

X number of years later, a very single John is having second thoughts. Sherlock, just as single, isn't.
