Because in my headcanon, Sherlock is so insistent that he's clean because he hadn't been a few months ago
Almost immediately after being invalided home, against medical advice, John leaves the hospital, sick and weak and barely able to move his left arm, for the halfway house. One day he's aimlessly wandering the streets and stops a sweet old woman from being mugged by a group of thugs. Fighting them off takes so much out of him that he practically loses consciousness, and the halfway house is farther away, so the sweet old lady takes him for a lie-down at her flat on Baker Street.
After he's had a rest Mrs. Hudson offers him a reduced rate as a thank-you. Only thing is, it would be with her new tenant. "He's really a sweet boy, on his good days. Though he's not been very well since coming out of rehab."
Basically, weak-and-sickly!John moves in with shaky-withdrawing!Sherlock at Mrs. Hudson's insistence. They swap hospital stories and help one another recover through ~the power of love~.
