Dont know if you'll have read these but:

Blackout by ivyblossom. // A short post S3 fic in which john copes with everything that's happened to him by becoming a blackout drunk. I love this fic. It's spare, deftly written, and disturbing. It's short but it packs a punch.

One Day at a Time series by KeelieThompson1. // After Sherlock's "death" John finds Sherlock stash of cocaine and one thing leads to another. This fic depicts very serious drug addiction with all the "worst case scenario" elements that often go along with it.

A couple of H/C recs:

Lost Without My Blogger by starrysummernights. // A long hurt/comfort fic, in which John is kidnapped and badly tortured, and Sherlock takes care of him with the utmost patience and consideration.

Improbable Reality by flawedamethyst. // In which John is trapped in a state of constant and bizarre hallucinations, but Sherlock refuses to give up on curing his friend.
