Castiel has a feeling Dean does not realize how on-edge Sam is, how prepared he always is for an onslaught. Castiel suspects it has less to do with his illness and more with the people he has lost. He suspects Dean does not need to know.

I kind of love your Cas POV because he's just so insightful, about all the things the Winchesters kind of take for granted about each other.

Castiel has possibly never seen Sam hopeful before.
