I'm thinking some of the people
here are reading too much into the picture.
I could see how one might think it has a message, but I've seen enough similar material that to me it isn't any different than something like
Muki's Kitchen: there to fulfill some sort of sexual fantasy.

"These days its getting harder and harder to distinguish between satire and seriousness. Thirty or forty years ago, it might have been seen as a commentary on the treatment of women, but today it is probably just intended to shock or to appeal to those who enjoy this kind of thing."
The ones that are reading a lot into it might be old folk, or psychology students, you have to admit, it's an image you can have a discussion about.
