Well, that's what this says. "ozomashii" (disgusting; repulsive). I don't quite agree with that though.
In the anime he describes his true form using the words "buzama" (unshapely; unsightly; clumsy; unpresentable; uncouth), "shuuaku" (ugliness; meanness; offensiveness), and "egetsunai" (dirty; vulgar; nasty).
Personally, I'd go with "Unbecoming...offensive...vulgar". This is because of 1) his dialogue with Ciel about how he didn't want Ciel to see this form, which made it sound (for me) less like it was frightening/ugly and more like it was...inappropriate (that is, it didn't mesh with his perfect, respectable butler image). And 2) the flashes of him we get make it look like he doesn't physically change all that much.
Also, I know that, when the episode first aired, the use of "ugly" [shuuaku] gave some people pause, but it's worth noting that he uses the same word to refer to Grell in the Jack-the-Ripper fight. I don't really think he uses it to refer to physical unattractiveness.
