...How does one tell if a toe is just painful when touched/walked on, swollen, bruised and generally weird-feeling; or if it's, you know, actually broken

When you move it, does it hurt? Does it hurt a lot? Does it make noise when you move it (snapping, clicking, cracking)? Can you move it at all? Is it deformed or just different in shape and size and temperature from the same toe on the other foot? Do you have a fever? (Check.) Can you stand on it?
Yes, depends if it catches me off guard; not that I can hear; a bit; Mmm... The shape/size/temp. option; I don't have a thermometer, but I don't think so? and Yes!
Really, the pain has gone down from when it was really bothering me, and I haven't taken any painkillers, so hopefully that's a good sign.
Doctors can't help a broken toe, anyway. All they'd do is tape it to the one next to it if it was REALLY hurting badly when you walked. But they can't really set it or anything - it pretty much has to heal on its own.

all you do is tape it up and wear good shoes.

I'm... fairly sure it isn't broken, just really badly bruised. But it's good to know that if, against the odds, it is, there's not much to do. *grins*
