COMPLETE ALL REQUIRED DOCUMENTS:

-VISA APPLICATION FORM
-ITINERARY
-BIRTH CERTIFICATE (issued by PSA within 1 year)
*IF LATE REGISTERED (2 PAGES BIRTH CERT), PROVIDE ELEM OR HIGH SCHOOL FORM 137/TRANSCRIPT OF RECORDS & BAPTISMAL CERTIFICATE
*If there is no record of birth in PSA, submit Birth Certificate issued by Local Civil Registrar and Negative Certificate issued by PSA.
-MARRIAGE CERTIFICATE (IF MARRIED)

-BANK STATEMENT
-PHOTOCOPY OF INCOME TAX RETURN (BIR FORM 2316)

Of course there would be a visa handling fee for the travel agency of your choice.

FOR VACCINATED TRAVELERS:

-VACCINATION CERTIFICATE (VAXCERT PH - free)/
YELLOW CARD (BUREAU OF QUARANTINE - 370 pesos, pick up - nearest satellite office or main branch in Manila)

For Muntinlupa vaccine card holders, I am not sure if your data is already uploaded in their system, so I suggest that you book an appointment with Bureau of Quarantine instead to get your yellow card. It would save you time instead of waiting for the VaxCertPh office in Muntinlupa because, apparently, their system has an issue. You'd have to pay 370 for BOQ but it would be much easier than to wait. I am not sure with the other LGUs, but I hope their system is better.

Disclaimer: This is only my experience. I was vaccinated in Las Pinas, got my first booster shot in Muntinlupa, and my second booster shot in Las Pinas. My 1st and 2nd vaccine shots were uploaded to the system, as well as my 2nd booster shot, except for the Muntinlupa one.

- MySOS (app - which I think would not be used now. Unless you've already uploaded your details before - around October)

If you're using a windows PC/laptop/android phone - I suggest you use Microsoft edge/internet browser of your phone. I don't know why, but using chrome was a hassle. It just doesn't work well in chrome.

I suggest that you upload all necessary information a week or more prior to your flight. This would save you a lot of time in waiting for email confirmation for each step!

NON-VACCINATED TRAVELERS:

-SUBMIT A 72-HOUR RESULT OF A PCR TEST PRIOR TO YOUR FLIGHT

- UPLOAD THE PCR TEST RESULT TO THE FAST TRACK

I'm not really sure how this works because I'm vaccinated, and they said that without vaccination cards and the fast track record would result to a lot of waiting in the airport.
