Yes, you do need an account.
I forgot how the registering process is, but if you go to the main page, there should be an orange button that says "Sign up for Pixiv". After you click it, it will ask for your email address. And I don't know the rest of the steps...

I don't know Japanese that well but I can read some of it, so if you need help, let me know. (Unless you already know Japanese yourself~)

I got to the step where you type in what you want your ID to be, but I don't know what it's asking me for underneath of that. I'd copy-paste it but Japanese characters don't show up on my computer. :[
