Yeah, it's a joke that she's the only one who can tell them apart (it's also why they like her).
This is SO COOL.
It wasn't her nature to get upset over the little things, but Tamaki was capable of excessiveness. Even her placid personality could be shaken into irritation by his overblown ego and grandiose ideas.
That's the best explanation of Tamaki/Haruhi I've ever heard! Some other details I liked were Haruhi not having the confidence to apply to an escalation school, Kyoya's translation of Tamaki's grand pronouncements, and Nokoru's fan.

I don't think I'm continuing this, since the joke can't run THAT far... but Tamaki and Nokoru are WAY TOO SIMILAR not to make the point.
