Did you notice that half of the things in your list of distractinos are games? You know where that question is going...

To get into the "working mode", I look at all of the work that I have to do and then I sit down with the intent of knocking things off that list and getting as far as I can. More importantly, I do not allow myself to take breaks until I have been sitting there in work mode for at least a few hours.
