I want to be sure I'm clear about the 'PMB 297' part. If I was line-breaking this in, say, a web order form so I can directly ship you a POD copy, would I be entering this as:

City, State, Zip: Chicago, IL 60615

Or is that something more like

Recipient: Kenneth Hite

More like the second; PMB stands for Private Mail Box.

But the people at my PMB are on the ball, so if you accidentally do the first, it will likely still get to me.
