Seriously, it sure took Dean long enough to take some Apocalyptic responsibility. I don't doubt that he beat himself up over it (post-"On The Head Of A Pin" where the self-hating angst was obvious), but all we've seen is a lot of finger-pointing in Sammy's direction until now. So, finally!

It has to be from several episodes, which is usually the case with the SOON promos. For some reason, I can't remember the terminology they use for single episode promos (probably something clever like "on the next, all-new Supernatural...!"). That would be one clusterfuck of an episode otherwise. :)
