Oh, so well done! And of course the thought of Dean would be his lifeline, no matter what is being done to him - even though Dean wasn't really in the story, you managed to show their relationship so well.
An interesting piece. I liked that Sam, trapped in his unending dream, had it always being Dean coming to his rescue, even when he didn't know it *was* Dean.
I liked these lines, especially:

I don't comment enough on your writing, but seriously this one blew me away. SO heartbreaking.
I just hate sam without dean, and I hate that he could have almost everything and still be empty without his brother.
