I'm sure - or at least the likes of Hawkeye Pierce have suggested to me - that regularly practicing doctors don't ordinarily allow themselves to get so involved with their patients, or they'd get wrung up and burnt out in no time. But Martha hasn't ever had a regular practice - she traveled with the Doctor just before her exams, and after them she ended up in UNIT where, certainly in Sontaran Sky, her duties appeared to run more to command and organization, even if she got to do some real doctoring while she was seconded to Torchwood between her TARDIS times. I'm sure she got the lectures during school, but she may not have had enough experiences like this one to develop the required detachment to offset her natural compassion. It wouldn't be in character for Leonard McCoy to react like this, but it is for Martha.
Every Doctor has to deal with this and have to move on quickly to the next patient. So Martha is lucky to be able to mourn her patient and have the Doctor there for her. Great fic.
