This whole episode made me so very sad for Dean on, like, a fundamental level.
Dean is a hunter. Not because it's his destiny or whatever, but because he simply knows no other way to be. That's all he's ever known - it's all he's ever wanted to know.
In his alternate reality - all the 'dreams' are Sam's. Sam is at Stanford, Sam is with Jess, Sam is going to be a lawyer. They're Sam's dreams, not Dean's.
Once upon a time, Dean said he'd always wanted to be a fireman. But in that reality, he was a mechanic.
Dean is so wrapped up in Sam that as long as he is happy, Dean is too.
