Ouran have many pic of the club together, is it OK for the icon represent not only pair (2 persons) but 3 ? (like Haruhi/Tamaki and a bit of Kyouya's shoulder/face)

If you have chosen to icon "general pairings" from a series then you may include some "OT3". Or if you have chosen to icon an OT3, then yes you may include 3 characters.

However, if you've chosen a specific pairing, like the Harhi/Tamaki example you give above (I'm sorry, I am not familiar with the series) then I personally don't see why a third person should be included. That's my view on it for now, hope it clears things up.

Oh yes, OT3 is fine but again, you can only have up to 4 icons of the one OT3. Like in Bleach if you're going for Nnoitra/Nelliel/Tesla you can only have up to 4 icons of those three. :)

If you make a general pairings claim, we will only accept up to 4 icons of any given pairing so mix it up.
