I'm starting to wonder if the writers are going to look at Sam's use of his powers (and let's face it, he's not going to give those up willingly) as a struggle with addiction. And while I have some serious qualms about his continued association with Ruby, how I feel about his abilities depends on whether they were something he was born with those powers, or if they are a product of the YED's blood. (Kripke, could we get an answer on that STAT, please?) One thing that worries me about Sam's new skill: If he can pull a demon out of a body, can he pull an angel out as well? What about a human soul?
My suspicion of Castiel's inhabiting of the body is that it's previous owner has left the building, so to speak. If an angel's presence can burn out eyes, what would co-habiting do?
This ep definitely landed in the Top 5 for me and the interaction between Dean and Castiel were perfect.
I'm still struggling with the Sam and Ruby relationship. Despite the appearances I cannot believe that Sam would sleep with Ruby. She is, afterall, inhabiting an innocent girl's body (unless she was a willing vessel somehow..hhmmm..might have to ponder that one more) and it would be in effect rape. Plus Sam says he doesn't trust her. I can't believe he'd put himself in that vulnerable a position. I'm hoping there will be flashbacks that reveal how Ruby got Kristy's body and how they ended up together in the same room. It could be as simple as they took down some evil thing and it got messy and they were just innocently washing off the mess. Yeah...that's my story and I'm gonna stick with it!!!

There’s not much more that I can say. Except that I hope Sam talks to Dean about the whole Ruby thing sooner rather than later!
