Did you even watch that scene? He didn't *torture* the kid (or, vengeful serial killer!), seeing as the pain was a side-effect of checking his soul - a NECESSARY action in order to find out the buyer and save the soul. It's like going to the doctor: there's no life-saving surgery without pain.
Those were just words spoken in exaggeration, that wasn't what took place on screen. Saying that Castiel "tortured" that kid is like saying doctors are "butchers". He didn't inflict pain on purpose, the pain was a side-effect of the soul-checking procedure.
Sorry, I can't take anything *stated* on the show at face value if it doesn't agree with what I see on screen. Otherwise, I'd believe Sam was a monster when Dean accused him of being one, etc.
No, I agree that the show depicted Castiel checking the kid's soul and the kid suffering excruciating pain as a side-effect.
I also agree that the show made Dean clearly exaggerate speaking of "torture" to highlight how domesticated he had become during the past year.
Unwilling? It was an emergency procedure, just like wheeling someone into the ER, so being willing or unwilling to be subjected to an emergency surgery is besides the point.
