Had to take a picture of the form I filled out at Katie's pediatrician appointment today. Sorry for the quality, phone and running away toddler don't make it easy. Other than the "parents are" question, you can also see under my palm "mother/ guardian 2", before that it says "father /guardian 1". Gotta love my doc

А я видел у нас в Миннесоте, в буклете про страховку такое понятие как Registered Same-sex Domestic Partner.
