De-aged Sam doesn't get why he can't leave the bunker, and Dean can't properly explain to him that he's vulnerable. He's not a badass hunter anymore, he's a scrawny little kid, and the monsters out there know it, too.
Sam gets out somehow anyway, because he's stubborn and won't take no for an answer, and Dean's worst nightmare comes true.
Sam gets hurt on a hunt - a gash from a monster that leaves behind poison. Dean has to fix him up under less than ideal circumstances. The poison is doing a number on Sam - mentally and/or physically. Maybe he hallucinates, maybe he thinks he's still a kid, maybe he thinks Dean is his dad, whatever floats your boat. They have to wait out the poison's effects, or call someone for help.
Sam's a teenager. He's sick. Something is wrong with him (writer's choice, as long as he doesn't die from it), but his dad and brother are up to their eyeballs in a hunt and he pretends he's fine so he doesn't get in the way. He can only get by with that for so long.
