Hey! This sort of got away with me and became more about angst and brother feels than H/C over a back injury but I used that as a segue to the boys relationship in S9 and the effects of Gadreel’s possession. I hope you don’t mind! This also wasn’t beta’d so all mistakes are my own.

“Sam! Sammy!”
Sam groaned at the loudness of his brothers voice and wished he’d just shut the hell up. It was too early to be yelling in his ear like a banshee.
Sam attempted to roll over and burrow under his nonexistent covers when his whole body seized. His jaw snapped shut so hard he could taste blood as a wave of intense pain took hold of him and locked all his muscles.
“Damnit Sam, don’t move I told you that.”
Well for one you didn’t fucking tell me that my back was broke into two halves. Sam thought gloomily to himself until the pain subsided to a dull ache and he could breathe again

This time Sam seized up for a while different reason. The mere thought of moving was making Sam’s stomach queasy. He didn’t think he wanted to test that theory out but they couldn’t risk calling an ambulance to the bunker.
“I’ll go slow,” Dean promised softly. Sam knew his brother would never intentionally hurt him. He knew that.
However, too many things have passed between them. Too much hurt, pain, and bad blood.
They never meant to but they always somehow wound up hurting each other and Sam didn’t know how to escape it either.
It surprised Sam when he felt himself being slowly listed off the bed. He groaned and despite his best efforts tensed up.
“Don’t tense up,” Dean warned. “You’ll only hurt yourself more. Try and relax for me.”
Easier said than done. Sam thought. He heard Dean curse when his brother examined his back and he didn’t dare ask how it looked. He knew it wasn’t good otherwise they wouldn’t be in this predicament that they were in
