Sam was often more clingy when he was sick or injured when he was just a kid, so it was usual for Sam to do so when he was a full grown adult. It's not like Dean can complain anyway, he prefers if Sam sticks close by, just in case of course.

I'll throw in another vote for "Tiger"! And the "kiddo" at the end was perfection.
I loved Dean slowly realizing how bad it was, and then finally abandoning the food to attend to Sam.
And that shower scene was so fantastic. Dean getting excited by the mere idea of being in the shower with him, and then immediately putting Sam first and offering up some humor to relax his clingy, nervous brother.
At first Dean doesn't think anything of it.
