Sam, Dean. The brothers are on a hunt and stranded somewhere. Sam is seriously injured. They don't have their first aid supplies, because Sam lost his pack, and all Dean can do is sit with Sam, try to keep him warm and hydrated and watch his brother deteriorate.

Whether or not Sam survives is up to you.

Sam's wheezing, liquid gasps were the worst sounds he's ever heard.  Worse than that banshee's scream when they destroyed her home; worse than that water wraith's screech when they hacked off its head; worse than the wendigo's shriek when Dean had lit it up with a flare gun, but not before it had shredded Sam, gutting him from stem to fucking stern; worse than his own shuddering breaths as he struggled, oh god, how he struggled, to keep himself and Sam alee of the howling wind.

The pack holding their supplies, including the gauze, the spare water, and the goddamn shock blanket, had tumbled down the ravine when the wendigo had launched itself at Sam. Dean was an improviser, though, he'd dragged Sam - Sam who was wheezing, gasping, futility trying to hold back the stem of blood - back to the mouth of the wendigo's lair, wrapped him up in as much clothing as he could spare, and held on
