whats the likelihood of it was always planned but intentionally not included in reese's script to then pressure her on set where unlikely to waive it off? cuz then dont you have to set up shots, set the whole scene? or is it more normal to improv a whole new scene and thats just how movie sets work sometimes? like i know dialogue and basic actions can be improvised on the spot but im assuming this situation was just a whole new scene.

It depends a lot on the writer/director. There are some who are more rigid about sticking to the script but there are some who are very open to improvising, suggestions, etc. (especially if it doesn't require new camera/lighting).
But I think there are definitely some people who deliberately try to add scenes like this during the filming process so that it's harder for the actors to say no. It's shitty for an actor to read a script, sign on to a project, and then get asked to do things that weren't in the original script, especially when it comes to unnecessary nudity or sex scenes.
