So he only considers living together to be romantic and being married isn't? He feels like he's married, only if he ends up in the hospital, she will have no rights as a spouse to do anything or sometimes even be let into the room.

There are a lot of differences between living with someone and being legally married. He may be too dumb to be aware of them.

Not sure what hospital you've been to, but it's up to each hospital to determine that, it's not a law in the US. They can keep a non-married person out of the emergency room or a hospital room if that's their policy, and only let "family" members in. Any unmarried person also has no say in medical decisions while a patient is unconscious for example ( ... )
