How did you make and measure your gauge swatch? That may make a whole world of difference.
Typically you cast on 4" of stitches with a few extra for border rows knit in seed st. Then you knit a few rows of border, about 4" of stockinette with a few border stitches on the right and left. Then you wash and block your swatch according to how you're going to wash and block your garment. THEN you measure.
1 stitch over 21 can make a huge difference, depending on the number of stitches per inch and the number of stitches cast on. If the gauge is 21 stitches per 4", and you get 20, your item will be 1" bigger every 84 stitches.
