I'm think in particular Hot Zone and the one where he turned into a wraith. There are others, but as stated above I can't be bothered to actually look them up and quote stuff.

In Hot Zone it seemed obvious he thought he knew better than she did and was going to take charge and do it his way no matter what. He turned out to be massively, massively wrong, but the disturbing thing is that I don't see him learning from these mistakes.

Also he never seems to be punished for them. I don't remember him facing any sort of punishment for his behavior after Hot Zone. I think that's why he doesn't learn, no one tells him that it's wrong.

Exactly. Weir had her moment of "I told you so", but there really wasn't a reprimand. Certainly not like the one she gave McKay in "Trinity".

Agreed. Yes, so he saved a lot of lives in Hot Zone but a member of the medical staff who were legitimately walking the corridors could have activated the EMP. I suppose it's difficult for Wier to reprimand him as the highest military officer but he still comes under her jurisdiction and precisely for that reason she should define herself as the ultimate authority in such a crisis situation.

What really got my goat was him making Bates choose.
